Welcome to the Ambulnz Settlement Website.
A proposed Settlement has been reached in a class action lawsuit titled Maria Ballesteros v. Ambulnz Case No. 2025-005051-CA-01 (“Lawsuit”) filed in the Circuit Court of the Eleventh Judicial Circuit in and for Miami-Dade County, Florida.
This Lawsuit arises out of a cybersecurity incident that occurred in or around April 2024 (“Security Incident”). Ambulnz denies the allegations and all liability or wrongdoing with respect to any and all facts and claims alleged in the lawsuit but has agreed to a Settlement to avoid the costs and risks associated with continuing the case.
You are included in this Settlement if you are a Settlement Class Member. A Settlement Class Member is an individual who resides in the United States and whose personally identifiable information (“PII”) and/or personal health information (“PHI”) was potentially exposed to unauthorized third parties as a result of the Security Incident.
If you are a Settlement Class Member, your rights are affected whether you act or don’t act. Please read the Notice carefully.
Settlement Class Members may submit a Claim for the following benefits from the Settlement (1) 1 year of credit monitoring; (2) Ordinary Loss Claims – up to a total of $400 per claimant; (3) Lost Time - $18.50 per hour for up to 4 hours (for a total of $74); and (4) Extraordinary Loss Claims – up to a total of $4,500 per claimant. Members may submit a claim for an Alternative Cash Benefit of either $30 or $10 per claimant, depending on whether Ambulnz’s records reflect that their Social Security number or Driver’s License information was implicated in the Security Incident.
If you are a Settlement Class Member, you must submit a Claim Form to receive any benefits by September 15, 2025. The easiest way to submit a Claim Form is online here or you can complete and mail the Claim Form to the mailing address of the Settlement Administrator.
The Court in charge of this case must still decide whether to give final approval to the Settlement, including Co-Lead Counsel’s request for an award of attorneys’ fees and expenses. No Settlement benefits will be provided until the Court approves the Settlement and it becomes Final.
